As An Athlete
To become a Hamilton County, athlete, you must meet these requirements below:
Eligibility Athletes must be at least eight years old and meet one of these criteria:
- Identified by an agency/professional as having an intellectual disability; or
- Have a cognitive delay (learn slower than peers) per standardized measures; or
- Have significant learning/vocational problems due to cognitive delays requiring specially-designed instruction.
Athlete Forms & Registration
To participate in Special Olympics Hamilton County, all athletes must have current forms on file and be approved by the Hamilton County office.
Athlete Intake Form: Athlete Intake Form 2025
- Required for ALL athletes
- New forms are required every 3 years
- Must be completed by the athlete, parent, guardian or caregiver
Annual Registration Renewal Form: Athlete Annual Renewal Form
- Required annually to update athlete information, emergency contacts and medical information
Athlete Medical Form: Athlete Participation Medical Form (updated 2024)
- Required for ALL athletes
- Must be fully completed and signed by licensed medical professional (MD, DO, NP, PA, etc)
- Official form only—no substitutions will be accepted
- Physician may attach extra pages if needed
- New Medicals are required every 3 years
WHO NEEDS WHICH FORMS?
New Athletes:
- Athlete Medical Form
- Athlete Intake and Code of Conduct Forms
Existing Athletes with an expired medical:
- Athlete Medical Form
- Athlete Intake Form
- Code of Conduct Form
Current Athlete with a valid medical and intake forms on file:
- Annual Athlete Registration Renewal Form
FORM VALIDITY
- Athlete Medical Forms are valid for 3 years
- Athlete Intake Forms are valid for 3 years
- Athlete Code of Conduct Form is valid for 3 years
- Athlete Registration Renewal form is valid for 1 year
- Athlete Medical, Intake and Code of Conduct Forms must be submitted together. Individual submissions will not be accepted

- NO PHOTOS OR SCREENSHOTS WILL BE ACCEPTED
- Include all pages of the form and any supplemental pages completed by the physician

Please note: Mailing forms may result in processing delays. Email submission is preferred and allows for forms to be reviewed more quickly.
KEEP A COPY FOR YOUR RECORDS
We strongly recommend athletes/guardians/families keep a copy of the athletes paperwork. Efforts will be made to give advance notice regarding expiring medicals, but it is the responsibility of the athlete/guardian/family to ensure the paperwork is up to date.
IMPORTANT INFORMATION
- Coaches will not be accepting athlete paperwork
- All paperwork must be submitted directly to the Hamilton County office for review and approval
- Forms will be reviewed by Debi Williams, Hamilton County Forms Coordinator

PARTICIPATION REQUIREMENTS
Athletes must have all required forms received, reviewed and approved by the Hamilton County office before they can register for or participate in Special Olympics Hamilton County activities.
